包皮过长跟包茎还是不一样的  你说的这个情况应该是属于包皮过长  但只要用手往后撸包皮 能露出龟头的话  就不是包茎  包茎的话是必须要手术的  否则会影响性生活的  
不过个人建议哪怕是只是包皮过长也还是做一下包皮环切手术比较好  一个性生活的时候提高愉悦程度  在一个对个人生理卫生也有好处  而且包皮环切只是个小手术  一般情况不会有什么后遗症或者危险（就是刚做了手术那几天比较难受 因为一勃起就胀痛  呵呵）
